PA State's
Required Health Testing per Grade:
The
Pennsylvania Department of Health mandates the following grade
level examinations :
Grade K Physical Exam, Dental
Exam
Grade 3 Dental Exam
Grade 6 Physical Exam and
Scoliosis Screen
Grade 7 Scoliosis Screen, Dental
Exam
Immunization update before entering seventh grade
Hepatitis B Vaccine Series (3 injections)
Varicella Vaccine (or provide age of child
with chickenpox disease)
Grade 11 Physical Exam
Other Examinations will be
offered for new students in other grades who have
not had a physical or dental exam on their school
health record.
The Department of Health encourages the family
physician and dentist to conduct the examinations
since they are aware of the students health
status and history. These examinations are at the
parents expense. If possible, private
examinations should be done over the summer to
have the forms completed before SEPTEMBER.
Physical and dental examinations done up
to one year before grade entry will be accepted.
An appointment may be scheduled with the school
physician and/or school dentist for any student whose
form is not returned to school. Parents/
guardians will be notified of the examination
date and must submit signed permission to
schedule an exam. These examinations are
free of cost.
All students, Kindergarten through Grade
12 will be given a visual screening test yearly
at school.
Hearing screening tests will be given in
Kindergarten and Grades 1, 2, 3, 7 and 11
All known hearing losses will be tested
each year at school. Parents will be notified if
any remedial condition is found.
***All students are
required by the State of Pennsylvania to have
hearing/vision/height and weight screenings
yearly.
Remember to take a state physical or dental
examination form to your doctor or dentist to
complete.
Send that completed form to the nurses'
office so they know the testing has been done.
If no form has been received, your child
may be put on the schedule for the doctor/dentist
to do the examination here at school.
